We are excited to announce the American Indian Integrated Health Care Forum V, which focuses on Integrated Health care in American Indian Tribal Communities. The theme for the 2018 bi annual event is Achieving Integrated Health & Wellness for Tribes in Arizona. This two-day event will be held on April 4, 2108 through April 6, 2018 at the Hon-Dah Resort Casino located at 777 Hwy 260, Pinetop, Arizona 85935. Your valued support is needed to make this event a success.
American Indians in Arizona experience significant health disparities and often have great difficulty in accessing quality health care. The AI Integrated Health Care Forum V is an event that will focus on health care needs of this population as an entire system of care: prevention, treatment, support services, and resiliency. The Forum will provide a space for healthcare professions to discuss their concerns, successes and challenges, and will provide tolls and best practices to all participants who attend.
This forum would not be possible without your generous investment in American Indian health. Additionally, you are improving the tools health care providers have statewide to increase the quality of life for the tribal communities they service, which provides benefit and decreases cost to all of us. Please return the completed form no later than March 15, 2018, so that we may recognize your agency in marketing and registration materials. Please note that sponsorships received after the date may receive reduced benefits due to printing and marketing deadlines.
