Exhibitor Booth Registration

Event location: Hon-Dah Resort Casino – 777 Hwy 260, Pinetop, AZ 85935

Exhibit set-up: Exhibitors will have the opportunity to set up at 7:00am on Thursday, April 5, 2018 through Friday, April 6, 2018 at 12:00 noon. The Arizona American Indian Integrated Care Planning Committee has the right to assign and/or change exhibit locations due to unavoidable problems or circumstances beyond the control of the parties involved. The committee also has the right to refuse exhibit applicants whose exhibit is not in the best interest of the event.

Exhibit space: Includes one table and two chairs. Please contact Julia Chavez for special needs.

Indemnifications: The Planning Committee along with the Hon-Dah Resort Casino/White Mountain Apache Tribe will not be responsible for any injury, loss, or damage that may occur to the exhibitor, the exhibitor’s employees, property, or to any other person prior to during or subsequent to the period covered by this registration form.
